– Adult Day Program

The Adult Day Program (ADP) is a multilingual service that offer social and recreational activities for people with physical or cognitive impairments.

Individuals are cared for in a friendly and secure environment where we strive to keep everyone socially active and mentally and physically stimulated. The program also provides the opportunity for caregivers to have time to themselves and manage the stress of providing care.

We also offer an Enhanced Adult Day Program for frail, at-risk older adults.

COVID-19 update: While our physical program is currently closed, we are offering virtual activities through our “ADP at Home” program. If necessary, we have support staff available who can come to your home (following strict COVID-19 protocols) and help facilitate your virtual experience.

 

Essential Information

What does the Adult Day Program consist of:

Our regular, in-person program (currently paused) includes —

  • Activities such as arts and crafts, yoga, memory games and gardening
  • A daily hot meal, beverages and snacks
  • Assistance with personal care and feeding

Virtual activities currently operating as “ADP at Home” include —

  • Music
  • Gentle fitness
  • Chair yoga
  • Relaxation and meditation
  • Bingo
  • Personalized activation plan (which may includes recommendations for walks, books, arts and crafts, etc.)

Who is eligible:

Our programs are designed for older adults who are —

  • isolated and vulnerable with physical impairments
  • socially isolated
  • living with Alzheimer’s disease or other forms of dementia
  • recovering from stroke

We also have an Enhanced Adult Day Program for frail, at-risk older adults with moderate to high cognitive impairments, behaviour issues and/or complex chronic medical conditions. Individuals in this program must be referred by Community Care Access Centre (CCAC).

Where:

Our service area is from Bloor Street to Lake Ontario, Roncesvalles Avenue to Yonge Street. Our physical location is at 248 Ossington Ave (at Dundas Street West).

Our physical location’s hours of operation are —

Mondays, Wednesdays, Thursdays and Fridays: 8 a.m. – 6 p.m.
Tuesdays: 8 a.m. – 8 p.m.
Saturdays: 9 a.m. – 5 p.m.
